- TEAM MANAGER must register the team and add players (11 – 18 players) and pay team fee. Manager must have each team member’s first and last name, DOB, shirt size and email address. If issue adding a member, call 863-533-0055.
- Team Members entering other events in Polk Senior Games
should also register as an individual online or by paper. It is helpful in
processing if the Team Manager submits the team and all members before a member submits an Individual Entry Form. - NOTICE: DAY OF EVENT CHECK-IN – CHANGE FROM PREVIOUS YEARS
The Team Manager must submit a copy of the final roster along with a SIGNATURE OF EACH MEMBER AGREEING TO THE RELEASE OF LIABILITY. A roster/signature form will be emailed to the Team Manager after their entry is processed. SIGNATURES ARE NOT NEEDED WHEN REGISTERING THE TEAM.
- OPTION 2) PAPER REGISTRATION – DEADLINE 5:00 PM ET on Friday, Feb 6
- TEAM MANAGER must complete the Team Entry Form. Each member’s name, date of birth, shirt size and email address must be included to process registration. Signatures are not required until event day.
